Silent Hill f unfolds in the fictional Japanese town of Ebisugaoka, inspired by Kanayama in Gero, Gifu Prefecture. Okamoto explained, "Japanese horror often intertwines beauty with terror. When something becomes overwhelmingly beautiful and perfect, it can become deeply unsettling. Players will experience this through the eyes of a young girl facing a beautiful yet terrifying decision."

As a dedicated fan of the franchise, Ryukishi07 has played all the Silent Hill games. He views Silent Hill f as both a return to the series' roots and a fresh challenge. One significant hurdle was creating a Silent Hill game outside the series' traditional setting.
